Payment Authorization & Terms

Authorization
By submitting payment you authorize HealOx Health PLLC to charge your payment card a single, one-time amount equal to the total shown at checkout (your selected package plus the 3% service fee described below).
Service fee
A 3% service fee is added to your order. The fee is shown as a separate line in your checkout total and on your receipt.
One-time only — no stored card, no recurring
Your card is processed by Stripe and is not stored by HealOx Health PLLC. There are no future, automatic, or recurring charges without a separate, explicit authorization.
Statement descriptor
This charge will appear on your card statement as
HEALOX HEALTH.Receipt
A receipt is emailed to the address you provide at checkout.
Refunds & cancellation
You may request a refund equal to the amount paid minus $350 for each HBOT dive (session) already completed. Once the number of completed dives multiplied by $350 equals or exceeds the amount paid, no refund is due. The 3% service fee is non-refundable.
Cardholder authorization
You represent that you are the cardholder or an authorized user of the card.
Disputes
Please contact us at billing@healoxhealth.com or 855-787-8128 before disputing a charge with your bank; we will work to resolve any issue.
Amount & currency
All amounts are in USD. The amount charged equals the displayed package price plus the 3% service fee. No sales tax is added.
Card security
Card data is collected and processed by Stripe (PCI-DSS Level 1). HealOx Health PLLC never sees or stores your full card number.
Electronic acceptance
Checking the authorization box and submitting payment constitutes your electronic signature and your acceptance of these terms.
